i think its for pictures that kinda look the same but are different enough to not be repeats, hence the squiggly equal sign

On just one occasion, I actually selected "Similar, keep both". The picture was exactly the same, but one had a better title while the other had a better caption. Overall, it's usually pointless to include "similar, keep both"as an option.
